    On 16/06/08, Covidien France withdrew from the market all batches of the references of the medical devices called Gels for ultrasound and ECG contact gels, non-sterile mentioned in the recall (16/06/2008) (36 (ko) attached, following the detection of bacterial contamination of an ultrasound gel sample. Covidien France has directly notified the recipients of the offending lots by means of the enclosed message validated by Afssaps. This information is addressed to the directors and correspondents of materiovigilance for diffusion if necessary to the services concerned. The relevant European Competent Authorities are informed of this measure by the manufacturer.

    “If our surveillance systems identify a potential performance issue, our personnel promptly evaluate the problem, including, when appropriate, conducting root cause investigations and internal testing to assess whether the product continues to meet specifications and defined performance criteria,” Medtronic told ICIJ in a statement. “In some cases, based on this evaluation, Medtronic may determine that a recall is necessary.” The company said that it communicates with healthcare providers and/or patients and provide recommendations to address such issues. Medtronic noted that these communications can include letters, emails, calls, press releases, physician notifications and social media postings, as well as informing the FDA and other regulators of the actions.
